The following items must be submitted by March 1, 2010 for the applicant to receive full consideration for a 2010 scholarship:

The on-line registration form must be completedand submitted automatically via email.

The scholarship application can either be completed as a WORD form and then printed, signed, and mailed; or it can be printed as a PDF document and then completed, signed, and mailed (separate pages for the responses and essay may be included).

A copy of the applicant’s high school transcript needs to be mailed with the scholarship application.

One letter of recommendation obtained from a non-relative adult that can attest to the applicant’s character, work ethic, and career interests also needs to be mailed with the scholarship application.

A head and shoulders photograph may either be mailed with the application or sent as a digital JPEG file to info@fcsillinois.com.
Mail the application, transcript, and reference letter (and photograph if it is not submitted via email) to:
